Objectif(s): The overall objective of this study is to demonstrate the feasibility of a community pharmacy-based approach to the management of hypertension in patients with diabetes residing in northern communities.
Description du projet: This license has been issued for the scientific research application #781. The overall objective of this study is to demonstrate the feasibility of a community pharmacy-based approach to the management of hypertension in patients with diabetes residing in northern communities. Eighty-two patients will be approached to participate in the study and the pharmacist will work with them to measure their blood pressure in the community pharmacy. Patients must be older than 18 years of age, able to learn and perform blood pressure tests on themselves and have either type 1 or 2 diabetes. Blood pressure will be assessed according to the recommended technique advocated by the 2007 Canadian Hypertension and Education Program (CHEP) and the 2006 CHEP guidelines for the management of hypertension by pharmacists. Three blood pressure reading will occur the course of the study with the pharmacist as well as education on hypertension and its management. The primary care physician or nurse will be included and notified of health concerns based on normal pharmacist/doctor interactions and confidentiality. The research results will be communicated to the individual participants, communities, regional, and territorial organizations via presentations (using lay language), and written reports. All information that will be collected about patients will be kept private and no information will be published that will individually identify patients. Pharmacies/pharmacists will be compensated for their clinical care services according to a fee schedule. The fieldwork for this study will be conducted from March 02 to December 31, 2009 in the Northern Grocery Store Pharmacy, Inuvik.
