Objective(s): Update existing Health and Health Resources documents to include Social Services; Design process for collecting, compiling and analyzing Health and Social Services data from Town of Fort Smith and subsequently to formulate conclusions; Define requirements of a healthy community from Health and Social Services point of view and to identify deficiencies; Document process and recommendations in a report to project steering committee.

Project Description: Proposed work includes 1) Update of community profile 2) assessment of community needs by way of questionnaires, public meetings, workshops, personal interviews & unstructured written responses; 3)develop a vision of community wellness based on community responses and current workable definitiions and 4) final report which includes: i)prioritization of Health & SS issues confronting the community ii) strategy to include program/service options for priorities
